RELEASE & CONSENT TERMS

Susan Stennis Layton, dba "Chiara," Service Provider, provides Spiritual Life Coaching, in-person, on-line, live and recorded sessions, groups, meetups, retreats & trainings, Reiki, Energy Healing, Mindfulness, Movement, Meditation, herein Services. 

*Services do not include psychological counseling, even though Service Provider is a licensed therapist.

BENEFITS & RISKS: Understanding yourself and your world around you from a spiritual perspective can help you lead a more meaningful life. As you clear out energetic blocks to living the life that you were always meant to live, you may discover that you have a profound purpose & that all of the events in your life have been orchestrated to bring you to your true path. A spiritual life coach can help you connect with who you really are & heal your old wounds so that you can move into a better, brighter future. Possible benefits include increased healing, success, stress reduction, relaxation, release of old patterns and finding balance in life. Risks may include feeling an increase of symptoms and changes associated with the healing process.

SESSIONS: Please be on time for your session so that we can utilize our full appointment time. If you are running late, please call or text to inform me if you expect to be more than ten minutes late.
NO EMERGENCY SERVICES: I am NOT available for emergencies. If you think that you are having a psychiatric emergency or need to speak to a mental health professional immediately, call the toll-free Crisis Hotline, or dial 911 or go to your nearest emergency room.
VOICEMAIL/TEXTING /INTERNET: We may communicate by phone, text via smart phone, Skype, Face Time or Zoom. While I take reasonable measures to ensure confidentiality of these communications, there is an inherent risk of the privacy of communications over cellular devices and the internet being compromised. Please do not text or email private content related to your sessions. These methods of communication are not completely secure or confidential.
CANCELLATION POLICY: If for any reason you are unable to attend a scheduled session, I request a 24 hour cancellation notice, so that I have adequate time to reschedule others in your reserved time. If I am not notified within the 24 hours prior to the scheduled appointment, you will be charged your usual fee.

I agree to release Susan Stennis Layton, dba “Chiara” from any liability associated with any Services provided by her.
